塞来昔布胶囊在骨科择期类手术镇痛中的应用

摘要(Abstract)：
目的研究塞来昔布胶囊在骨科择期类手术镇痛中的超前镇痛作用。方法将38例患者随机分为A组24例和B组14例。在术前48 h、24 h A组患者口服塞来昔布胶囊400 mg,对照组口服安慰剂。之后于术前2 h、术毕及术后2 h、术后6 h分别静脉采血,离心后取上清,采用酶联免疫吸附法(ELISA)测定上清液中环氧化酶-2(COX-2)和前列腺素E2(PGE2)水平。术后6 h、24 h、48 h及72 h采用VAS视觉评分法对患者进行疼痛评估,并对患者术后镇痛药物的消耗进行统计。结果术毕、术后2 h 2组PGE2水平比较有显著性差异,而COX-2水平比较无显著性差异。术后6 h 2组血清COX-2和PGE2含量比较无显著性差异。A组术前、术后血清中COX-2和PGE2含量比较无显著性差异;B组术前、术后血清中PGE2含量比较有显著性差异,而COX-2比较无显著性差异。术后2组VAS评分比较无显著性差异;A组塞来昔布胶囊平均消耗1 900 mg,B组平均消耗2 650 mg,2组比较有显著性差异。结论塞来昔布胶囊超前镇痛可明显降低术后血清中PGE2的水平,减少术后镇痛药物的消耗。
